Abstract: | Os pólenes de Poaceae, Olea e Platanus são importantes agentes etiológicos da alergia respiratória na região Mediterrânica europeia, pelo que o estudo do seu conteúdo na atmosfera é uma ferramenta útil no diagnóstico, tratamento e prevenção dessa patologia. Objectivos: Analisar a aerobiologia destes tipos polínicos e as potenciais repercussões na doença alérgica respiratória no Sul de Portugal. Materiais e Métodos: Utilizaram-se as monitorizações de pólen em 3 estações da Rede Portuguesa de Aerobiologia desta região do País, respectivamente Lisboa, Portimão (2002-2008) e Évora, (2001-2008), mediante a metodologia padronizada pela International Association for Aerobiology - IAA. Compararam-se os resultados das monitorizações dos pólenes de Poaceae, Olea e Platanus com os dados diários dos parâmetros meteorológicos e com os resultados das provas cutâneas e dos inquéritos alergológicos obtidos numa amostra de 142 doentes com polinose da Consulta Externa de Imunoalergologia do Hospital do Espírito Santo em Évora. As monitorizações de pólen também foram relacionados com as vendas mensais e anuais de antihistamínicos de Lisboa, Évora e Faro (2002 a 2007). Resultados: Estabeleceu-se o calendário polínico para cada uma das localidades. Os pólenes de Poaceae, Olea e Platanus mostraram uma boa representação no espectro polínico da atmosfera das várias localidades, com excepção do pólen de Platanus em Portimão. Observaram-se diferenças anuais na distribuição sazonal e intradiária destes tipos polínicos dentro e entre localidades, e obtiveram-se correlações significativas (p<0,05) entre as suas concentrações e os parâmetros meteorológicos. A prevalência da positividade dos testes prick dos doentes foi: Poaceae (99%), Chenopodium album (56%), Olea europaea (47%), Platanus occidentalis (38%), Plantago lanceolata e Parietaria judaica (34%). As vendas mensais de antihistamínicos (Maio-Julho 2002-2007) correlacionaram-se significativamente (p<0,05) com as contagens mensais desses pólenes. Não se detectaram doentes monosensibilizados aos pólenes de Olea e Platanus. A maioria dos doentes com sensibilização pertencia ao sexo feminino e sofria predominantemente de rinite alérgica. Conclusões: Entre as localidades registaram-se diferenças qualitativas, quantitativas e de comportamento aerobiológico, que resultam das diferenças na vegetação e das condições ambientais. O pólen de Poaceae é o que está mais associado à ocorrência de polinose no Alentejo e possivelmente em todas as regiões do Sul do País, sendo provavelmente responsável pelas variações interanuais nas vendas de antihistamínicos durante a Primavera; ABSTRACT:Airborne pollens of Poaceae, Platanus and Olea are important etiological agents of respiratory allergy in the European Mediterranean region and, so the study of its presence in the atmosphere is a useful tool in the diagnosis, treatment and prevention of this disease. Objectives: To analyze the aerobiology of these pollen types and its potential effects on allergic respiratory disease in southern Portugal. Material and Methods: Pollen monitoring data from the 3 stations of the Portuguese Aerobiology Network located in this region of the country were used, respectively Lisbon, Portimão (2002-2008) and Évora (2001-2008), through standardized methodology for International Association for Aerobiology - IAA. It was compared the results of the monitoring of airborne pollens of Poaceae, Platanus and Olea with daily meteorological data, with the results of skin prick tests and allergologic inquiries from 142 patients from the Outpatient Immunoallergology of Espirito Santo Hospital in Évora, diagnosed with pollinosis. Pollen monitoring was also related with monthly and annual antihistaminic sales in Lisbon, Évora and Faro (2002 to 2007). Results: For each locality a pollen calendar was established. Poaceae, Platanus and Olea pollens showed a good representation in the pollen spectrum of the atmosphere of each locality, with exception of Platanus pollen in Portimão. There were observed differences, within and between localities, in annual and seasonal distribution of these pollens types. Significant correlations (p < 0.05) between their concentrations and meteorological parameters were obtained. The prevalence of positive skin prick tests in patients was: Poaceae (99%), Chenopodium album (56%), Olea europaea (47%), Platanus occidentalis (38%), Plantago lanceolata and Parietaria judaica (34%). It was observed a correlation between the monthly antihistaminic sales (May-July 2002-2007) and the monthly pollens counts of these pollens (p < 0.05). Olea and Platanus monosensitized patients were not detected. Most of the patients with sensitization was female and predominantly suffering from allergic rhinitis. Conclusions: There were qualitative, quantitative and dynamic aerobiological differences between the localities, as a consequence of differences in vegetation and environmental conditions. Poaceae airborne pollen is the most associated with occurrence of pollinosis in Alentejo and possibly in all south regions, and probably, it is also responsible for interannual variations in the antihistaminic sales during the spring. |
