Aleitamento materno: diagnóstico de situação e medidas promotoras no serviço de obstetrícia do hospital de Beja

Abstract

O aleitamento materno é influenciado por diversos fatores entre os quais se destaca a aquisição de conhecimentos. Sendo um dos fatores mais críticos na prevalência da amamentação este relatório tem como finalidade a caraterização dos conhecimentos das puérperas sobre aleitamento materno, das experiências por elas vivenciadas e, consequentemente, a descrição de algumas medidas promotoras de aleitamento materno implementadas. A população-alvo da nossa intervenção foram as puérperas internadas no serviço de obstetrícia do Hospital de Beja. Na recolha de dados das necessidades da população-alvo utilizámos entrevistas e questionários. Os principais resultados obtidos nas entrevistas relacionaram-se com as dúvidas sobre o horário, duração da mamada, pega correta e superação de algumas dificuldades iniciais. A análise dos questionários permitiu-nos as seguintes conclusões: as puérperas reconhecem que o leite materno é o melhor alimento do recém-nascido; apresentam incertezas quanto à forma adequada de resolver a “subida” do leite e dúvidas relacionadas com o horário da mamada; não sabem o que fazer em caso de mastite; não sabem identificar sinais sugestivos de candidíase mamilar. As intervenções que desenvolvemos com vista à promoção do aleitamento materno no serviço foram: divulgação dos resultados da investigação qualitativa às colegas enfermeiras; realização de proposta escrita a solicitar o início do registo eletrónico do aleitamento materno na maternidade; elaboração e distribuição de folhetos informativos às puérperas. Esperamos que o relatório constituía um passo fundamental na motivação dos profissionais de saúde, principalmente das enfermeiras, no desenvolvimento de outras estratégias de apoio à amamentação. Breastfeeding is influenced by several factors among which stands out the acquisition of knowledge. Being one of the most critical factors in the prevalence of breastfeeding, this report has as purpose the characterization of knowledge of recent mothers on breastfeeding, the experiences they experienced and consequently the description of some breastfeeding promotion measures implemented. The target population of our intervention were recent mothers admitted in the obstetrics service of Hospital of Beja. In the data collection of the needs of the target population we have used interviews and questionnaires. The main results obtained in the interviews related to doubts about the time of feeding, breast correct handle and overcome some difficulties. The analysis of the questionnaires has enabled us to the following conclusions: recent mothers recognize that breast milk is the best food of the newborn; they show some uncertainty as to the appropriate way to resolve the "rise" of milk and questions related to the time of feeding; they don't know what to do in case of mastitis and also don't know how to identify signs suggestive of candidiasis nipple. The interventions that we developed to promote breastfeeding in our service were: communication of the results of the qualitative research to fellow nurses; conducting proposal writing to request the beginning of electronic registration of breastfeeding in maternity; preparation and delivering of brochures for the recent mothers. As far as we are concerned, we hope that the report constituted a fundamental step in the motivation of health professionals, especially nurses, to develop other strategies to support breastfeeding.
