Mecanismos de defesa e comportamentos suicidários: uma revisão sistemática de literatura

Abstract

Os comportamentos suicidários (CSs), expressões do espectro do suicídio, constituem um fenómeno complexo e multifatorial associado à dor psíquica e ao funcionamento defensivo. Esta revisão sistemática, conduzida segundo as diretrizes Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020, sintetizou a evidência sobre a relação entre mecanismos de defesa (MDs) e CSs, colmatando uma lacuna centrada sobretudo na dissociação. A pesquisa nas bases PubMed, PsycINFO e Scopus incluiu 34 estudos empíricos (1972–2025) que cumpriram os critérios de elegibilidade. A síntese narrativa revelou associações consistentes entre estilos defensivos imaturos e ideação suicida, tentativas de suicídio e risco suicidário, permitindo organizar os resultados em oito padrões defensivos. Destacaram-se a projeção, regressão, acting out, desvalorização e, sobretudo, a dimensão Turning Against Self. As principais limitações relacionam-se com a heterogeneidade metodológica dos estudos. Globalmente, os resultados sugerem que o funcionamento defensivo é relevante para a compreensão clínica dos CSs; - Defence mechanisms and suicidal behaviours: A systematic literature review Abstract: Suicidal behaviours (SBs), as expressions of the suicide spectrum, constitute a complex and multifactorial phenomenon associated with psychological pain and defensive functioning. This systematic review, conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines, synthesised the evidence on the relationship between defence mechanisms (DMs) and SBs, addressing a gap in the literature focused mainly on dissociation. The search of PubMed, PsycINFO, and Scopus included 34 empirical studies (1972–2025) that met the eligibility criteria. The narrative synthesis revealed consistent associations between immature defensive styles and suicidal ideation, suicide attempts, and suicide risk, allowing the findings to be organised into eight defensive patterns. Projection, regression, acting out, devaluation, and particularly the Turning Against Self dimension stood out. The main limitations relate to the methodological heterogeneity of the studies. Overall, the findings suggest that defensive functioning is relevant to the clinical understanding of SBs
