Abstract: | O presente relatório enquadra-se no plano curricular do Mestrado em Enfermagem em
Associação, Área de Especialização em Enfermagem Médico-Cirúrgica: a Pessoa em Situação Critica.
Tem como fim contribuir para o progresso da Enfermagem Especializada, através da reflexão crítica
sobre a prática sustentada na evidência, com enfoque no desenvolvimento de competências de
excelência, e na capacitação para a supervisão, governação clínica e gestão de equipas, visando
espelhar todo o percurso formativo enquadrado neste curso de Mestrado. Apresenta-se ainda o
Projeto de Intervenção desenvolvido em contexto de estágio, com recurso à metodologia de projeto,
sobre a temática “ISBAR: Comunicação Efetiva, Transição de Cuidados Segura”, que se insere na linha
de investigação “Segurança e Qualidade de Vida” e tem como intento geral a melhoria da segurança e
qualidade dos cuidados de enfermagem, através da implementação de estratégias para otimização de
uma comunicação eficaz na transição de cuidados de saúde; ABSTRACT:
Title: ISBAR, effective communication, safe care transition
This report has been written as a part of the curricular plan of the master’s degree in the science
of nursing, area of specialisation in medical and surgical nursing practice: the critical patient. It intends
to contribute to the growth of the specialised nursing activity, through critical reflecting about
evidence supported practice, emphasizing the development of skills of excellence and the training of
supervision capacity, clinical and team management, aiming to reflect the educational path framed
within this master’s degree course. The intervention project developed during the traineeship will also
be presented, applying the project methodology focused on the issue “ISBAR: effective
communication, safe transition of care, which makes part of the investigation “Safety and quality of
life” and aims to improve the safety and quality of the nursing care provided, through the
implementation of strategies to optimize the efficacy of communication regarding transition of health
care. |
