Abstract

Transporte Inter-Hospitalar do Doente Crítico Cardíaco com Necessidade de Intervenção Hemodinâmica O presente relatório surge no âmbito da conclusão do ciclo de estudos do Mestrado em Enfermagem, com o objetivo de descrever e fazer uma análise reflexiva do trabalho desenvolvido com vista à aquisição de Competências Comuns e Específicas do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica, bem como as de Mestre. Este foi fundamentado pela Teoria da Incerteza na Doença de Merle Mishel, concebida para abordar a incerteza da pessoa a vivenciar uma doença aguda ou crónica, dada a imprevisibilidade do curso da doença. No decorrer do Estágio Final, realizado numa Unidade de Cuidados Intensivos, foi desenvolvido um Projeto de Intervenção em Serviço, sob o tema Transporte Inter-hospitalar do Doente Crítico que culminou na elaboração de instrumento de trabalho/registo, Chek-List: Intervenção Hemodinâmica. Este documento, contendo campos com informações pertinentes sobre a atual situação clínica do doente, a preparação realizada para a intervenção a que será submetido e o registo de ocorrências durante o transporte, pretende contribuir para o aumento da segurança do doente crítico cardíaco com necessidade de intervenção hemodinâmica, aquando do seu transporte inter-hospitalar. A análise do processo de aquisição de competências reflete a incorporação e a mobilização de conhecimentos teóricos e práxicos, bem como o crescimento pessoal e profissional ocorrido durante todo este processo; ABSTRACT: Inter-hospital Transport of Cardiac Critical Patient in need Hemodynamic Intervention This report is part of the conclusion of the studies of the Master in Nursing, and aims to report and make a reflective analysis of the work developed with a view to the acquisition of Common and Specific Skills of the Specialist Nurse in Medical-Surgical Nursing as well as those of Master. This was based on the Theory of Uncertainty in Disease, of Merle Mishel, developed to approaching the uncertainties of the person experiencing an acute or chronic disease, given the unpredictability of the course of the disease. During the Final Stage, carried out in an intensive care unit, was developed a Intervention Project in service with the theme Inter-hospital Transport of the Critical Patient which led to the elaboration of a working/registration instrument, Checklist: Hemodynamic intervention. This document, containing fields with pertinent information about the current clinical situation of the patient, the preparation made for the intervention to be submitted and the record of occurrences during the transport, aims to contribute to increase the safety of critical cardiac patients with need for hemodynamic intervention, during inter-hospital transport. The analysis of the competency acquisition process reflects the incorporation and mobilization of theoretical and praxical knowledge, as well as the personal and professional growth that occurred throughout this process.
