Title: Validação de um método interactivo em contexto de ensino clínico

Abstract: Os estudos desenvolvidos no âmbito do ensino clínico dos estudantes de enfermagem têm-se revelado como gritos de alerta e de desânimo dos agentes envolvidos nesse processo, apelando à tomada de consciência de uma mudança significativa que urge introduzir nos contextos, cabendo às organizações pugnar pela utilização do conhecimento que se tem vindo a produzir. A fim de darmos o nosso contributo, com algo inovador, numa perspectiva de melhoria do ensino clínico, temos vindo a gerir a concepção de um método pedagógico interactivo (MPI) de matriz sócio-construtivista que apresentámos no ano lectivo de 1997/1998, durante o ensino clínico de 25 estudantes do 2.° ano do Curso Superior de Enfermagem, com a cooperação de duas professoras da Escola Superior de Enfermagem Dr. Angelo da Fonseca (ESEAF), hoje Escola Superior de Enfermagem de Coimbra. Posteriormente, durante três anos lectivos, 2001 a 2004, difundimos e validámos a pertinência do MPI com estudantes do 3° ano do Curso de Licenciatura em Enfermagem, onde cooperaram duas professoras da Escola Superior de Enfermagem de Viseu (ESEnfV), presentemente Escola Superior de Saúde de Viseu, e doze enfermeiros licenciados, nove pertencendo ao quadro de pessoal dos Hospitais da Universidade de Coimbra (HUC) e três ao Centro Regional de Oncologia de Coimbra (CROC), a desempenharem funções de orientadores pedagógicos dos estudantes da ESEAF. Neste contexto, a amostra envolvida foi constituída por 417 estudantes, sendo 268 da ESEAF e 149 da ESEnfV. Com este estudo descritivo, sequencial e compreensivo, onde seguimos alguns princípios e formalidades da investigação- acção, são nossos objectivos principais: descrever de forma sequencial a concepção e purificação do MPI, analisar as implicações do MPI na aprendizagem dos estudantes ao longo do ensino clínico, validar o MPI enquanto instrumento pedagógico estruturado e estruturante da aprendizagem dos estudantes em ensino clínico, e apresentar uma estratégia de perspectiva de melhoria do ensino clínico. Sendo possível olhar o ensino clínico de vários ângulos, definimos ainda alguns objectivos secundários para uma melhor compreensão deste estudo: verificar se existem diferenças entre as expectativas e as vivências dos estudantes do 3.° ano, das duas escolas, acerca do ensino clínico, quanto à sua preparação, envolvimento e medos; analisar a satisfação dos estudantes do 3.° ano, das duas escolas, sobre a orientação do ensino clínico com a utilização do MPI, a nível de relacionamento interpessoal, autonomia, responsabilização e supervisão; analisar a opinião dos estudantes do 3.° ano, das duas escolas, sobre o MPI em geral e sobre cada uma das técnicas que o constituem, e analisar a opinião das professoras e dos enfermeiros que orientaram os estudantes, quanto à utilização e implicações do MPI no processo de ensino e aprendizagem. Os dados foram recolhidos junto dos estudantes através de questionários auto-administrados antes e após o ensino clínico e por uma entrevista às professoras e enfermeiros orientadores, a fim de obtermos indicadores compreensivos do fenómeno em estudo. De acordo com os resultados ficou demonstrada a validade do MPI no que se refere à sua utilidade e adaptabilidade, podendo por isso ser apresentado como uma estratégia de melhoria do ensino clínico. Relativamente às expectativas e vivências dos estudantes quanto à sua preparação, envolvimento e medos em ensino clínico, as vivências excederam de forma positiva as expectativas, independentemente dos anos lectivos e das escolas envolvidas, apresentando os estudantes do género feminino expectativas mais positivas relativamente aos estudantes do género masculino. Todos os estudantes se manifestaram satisfeitos por terem utilizado o MPI em ensino clínico, enfatizando a autonomia e responsabilização que lhes foram proporcionadas no desenvolvimento da sua aprendizagem. Consideraram ainda que o MPI os estimulou à aprendizagem autodirigida, profunda e reflexiva, diminuindo o medo de estar em ensino clínico, sugerindo que o método seja extensivo a todos os estudantes. Também as professoras e os enfermeiros orientadores salientaram a importância da utilização do MPI na co-responsabilização, autonomia e desenvolvimento pessoal dos estudantes. Consideraram que este método introduz uma reorganização científica ao ensino clínico, com implicações na qualidade dos cuidados, decorrente do desenvolvimento dos estudantes e dos enfermeiros da prática . #### Abstract - The studies developed in the scope of nursing students' clinical teaching have been clearly "alert signs", from the agents involved in the process who request the awareness of a significant change in the contexts; it is the organization's role to fight for the use of the knowledge that has been produced so far. In order to give our contribution, with something innovative, and aiming to improve the clinical teaching, we have been conceptualizing an interactive pedagogical method (IPM), with a socio constructivist shape, that we presented in the school year 1997/1998, during the clinical teaching of 25 students, enrolled in the second year of the Nursing Degree; we had the cooperation of two professors of the Escola Superior de Enfermagem Dr. Ângelo da Fonseca (ESEAF), which is today the Escola Superior de Enfermagem de Coimbra. Eventually, throughout three, school years, 2001 to 2004, we diffused and validated the pertinence of the IPM with students of the 3rd year of the Nursing Degree, and had the cooperation of two professors at the Escola Superior de Enfermagem de Viseu (ESEnfV), today the Escola Superior de Saúde de Viseu, and of twelve graduated nurses; nine of them were board members of the Hospitais da Universidade de Coimbra (HUC) and three in the Centro Regional de Oncologia de Coimbra (CROQ; ali of them were students' pedagogical supervisora. In this context, the sample was constituted by 417 students, 268 of which were drawn from the ESEAF and 149 from the ESEnfV. The main aims of this descriptive, sequential and comprehensive study, which was also guided by some principles and formalities of the research-action, were: to sequentially describe the conception and improvement of the IPM; to analyse the implications of the IPM to the students' learning throughout clinical teaching; to validate the IPM as a structured pedagogical instrument and structuring of students' learning in clinical teaching and to present a strategy of improvement perspective of the clinical teaching. As clinical teaching may be regarded from different perspectives, we also defined some secondary aims for a better understanding of this study: to confirm if there are differences between the expectations and the experiences of 3rd year students, from the two schools, about clinical, teaching as to their preparation, involvement and fears; to analyse the satisfaction of the 3rd year students, from the two schools, about the orientation of the clinical teaching with the use of the IPM, in what concerns interpersonal relationship, autonomy, responsabilization and supervision. To analyse the opinion of the 3rd year students, from the two schools, about the IPM in general and about each of the techniques that constitute it; and to analyse the opinion of the teachers and nurses who guided the students, on the use and implications, of the IPM in the learning teaching process. ZD Data were collected for the students through self administered questionnaires before and after the clinical teaching, and through interviews to the supervisor teachers and nurses in order to obtain the comprehensive indicators of the studied phenomenon. According to the results the validity of the IPM was demonstrated in what concerns its utility and adaptability, therefore it may be presented as a strategy for clinical teaching improvement. Regarding the expectancies and experiences of the students as to their preparation, involvement and fears in clinical teaching, expectancies were positively exceeded by the experiences, regardless of the school years and schools involved; female students had more positive expectancies than mala students. All students showed satisfaction for having used the IPM in clinical teaching, emphasizing the autonomy and responsabilization that they were given in the development of their learning. They also considered that the IPM motivated them to a self directed, deep and reflexive learning, reducing the fear of being in clinical teaching, and suggesting that the method is extensive to every student. The supervisor teachers and nurses have also highlighted the importance of the use of the IPM in the co responsabilization, autonomy and personal development of the students. They consider that this method introduces a scientific reorganization to the students' clinical teaching, with implications, in the service quality, which results from the students and nurses' development.
